COVID-19 SCREENING
BY PARTICIPATING YOU ARE INDICATING TO US THAT THE FOLLOWING ALL APPLY TO YOU:
1. You (& other members of your household) do not have any of the following symptoms:
- cough
- Fever
- respiratory difficulties
- loss of taste or smell
2. You (& other members of your household):
- have not been diagnosed with COVID-19
- are not awaiting test results for COVID-19
-have not had a negative test result but still have symptoms
3. In the 14 days before the event, you have not been in direct contact with someone who has been diagnosed with COVID-19.
4. You do not fall into the ‘clinically extremely vulnerable’ category and have been advised to ‘shield’.
IF ANY OF THESE STATEMENTS DO NOT APPLY TO YOU, YOU MUST NOT ATTEND.
